Is your mew still in gen 3 format? If it is, trade your mew over to XD and take it to Mt. Battle; you should see the Mew Tutor there. Then follow the instructions here.

That page doesn't give the combinations you want; you may be able to try all of them yourself, SR'ing every time (you can save in front of the Mew Tutor).
