Remember Ben 10? I'm surprised I really haven't seen any references to that here...like, I'm 100% the Omnitrix aliens could be used as an inspiration on this forum.

Appearance: After coming out of the worm hole, Golem lose it hard Shell and had become more of a serpent-like figure. It’s body had become more smooth. For whatever reason, Golem is power by an electrical generator inside of it’s body. It still have the same facial structure and short arms as it older body. It’s Serpent body is made out of blue & yellow scales. It body share some similarities to Rayquaza.

Appearance: Upon arrival in Ultra Space, Primarina's ended up in a planet made of pure ice, but hard like steel. Because of this, Primarina's tail turned into an odd rapier like object, and it seemed to absorb the energy of the "northern lights" that appeared over the planets. Their hair turned white, while their skin turned pale blue.
 
I've got some more ideas. These are extreme WIPs, but I want to get them out before the Free For All period ends.

UB-110 Nova (Weezing)

Ability: Radioactive - Any Pokémon that spends a total of three turns on the field with this Pokémon becomes badly poisoned regardless of type or Ability. (HA: Levitate)

Description: Weezing entered a world where physics was drastically different from our own. Gravitational forces were much stronger in that world, and as a result, the gases inside Weezing were compressed into a star. The heat of the star inside Weezing caused its skin to blacken and, in some places, crack to reveal glowing gases beneath the surface. It can still expel gas like regular Weezing, but now the gas is superheated and radioactive, causing severe injuries to its opponents. It can also eject blasts of the radioactive plasma from the star inside of it.
Justification: Stars are basically just enormous balls of plasma formed from extremely compressed gas. Weezing is a big ball of gas. Also, since Weezing is associated with fire and explosions anyway, this is perfect for it since regular Weezing already has almost exactly the moveset a Pokémon like this would have.
Competitive Analysis: Weezing works well physically or specially. Its new move Supernova allows it to tear through all but the toughest physical walls, crippling them with poison or a burn afterwards, and Sunsteel Strike allows you to ignore pesky abilities that get in your way. On the special side, with access to Thunderbolt, Flamethrower, Sludge Bomb, Flash Cannon, and a host of others, it can be used to great effect as a special tank. Radioactive and Venoshock make a good combo provided you can last long enough to get your opponent poisoned. On the other hand, Levitate removes its now much more glaring Ground weakness.

UB-623 Unsealed (Golurk)

Type: Ghost/Electric

Description: The seals on Golurk's chest were lost when it went through the Ultra Wormhole, letting it shed its bulky and slow body and roam Ultra Space as a being of pure electrical energy. The souls bound inside Golurk's body, now freed from their physical prison, attack in conjunction with Golurk itself, increasing its power by leaps and bounds. The released souls have kept the automaton's massive fists, and use them and Golurk's newfound electrical powers to destroy anything that gets in their way.
Justification: The golems of Jewish mythology were said to be inferior to humans because they did not have living souls. On the other hand, the ancient peoples who made Golurk had no qualms about binding human souls with magic and inserting them into a clay body to make golems. When Golurk’s seal is broken, not only does its internal energy escape, but so do all the souls trapped inside it.
Competitive Analysis: A great special sweeper. With blistering Speed and a base Special Attack on par with Xurkitree, and access to No Guard Zap Cannon and Focus Blast, Golurk is a force to be reckoned with. A physical set with No Guard Dynamic Punch and Meteor Mash (and maybe Extreme Speed) is also viable, but that’s definitely not the kind of Special Attack you want to ignore.

UB-537 Resonance (Seismitoad)

Type: Electric/Psychic
Ability: Pitch Perfect - All sound-based moves used by this Pokémon are guaranteed critical hits. (HA: Psychic Surge)

Description: Seismitoad found itself on a planet filled with predators. It was forced to adapt to survive, and its natural ability to cause vibrations has multiplied a hundredfold. It blasts its opponents apart by using its newfound psychic powers to analyze their bodily structure, then humming at the resonant frequency of their bones, causing their bones to lose cohesion and the rest of their body to turn into a blob of flesh, ripe for eating. Its vibrations are so fine-tuned that it can even vibrate atoms and molecules in complex patterns to create heat, electrical energy or electromagnetic waves.
Justification: Nikola Tesla believed that, given enough energy, an oscillating machine could cause an earthquake or destroy a building just by tuning to the right frequency. This form of Seismitoad is based on the idea of resonant frequencies.
Competitive Analysis: Specially tanky and appreciably fast, Seismitoad can take a huge chunk out of a lot of things with always-crit Boomburst, Chatter, or Bug Buzz. Psychic and Thunderbolt are good STAB coverage. Parting Shot is a good move on any Pokémon, and Calm Mind is a good option to make it a scary special sweeper.

UB-327 Intoxicate (Spinda)
Base Stats: 79/97/97/97/97/103 (570)
Type: Poison
Ability: Neurotoxin - 20% chance to poison and 20% chance to confuse on contact. (HA: Tinted Lens)
Removed Moves: Nothing. Have I mentioned how awful regular Spinda is?
Added Moves: Sludge Bomb, Sludge Wave, Gunk Shot, Belch, Venom Drench, Knock Off
Description: Spinda was deposited in a dimension with various kinds of strange chemicals that reacted with its bodily fluids in unusual ways. As a result, these Spinda secrete a kind of chemical very similar to ethyl alcohol that can be absorbed through the skin and is a very potent neurotoxin. They are not completely immune to its visual-impairment effect themselves, and have therefore developed lenses over their eyes that automatically correct the visual distortions caused by the toxin's effects.
Justification: Regular Spinda looks and acts like it’s drunk. This form of Spinda secretes what is basically alcohol except even worse for you. The lens over its eyes are a reference to the term “beer goggles”, though their actual function is the exact opposite.
Competitive Analysis: Though it’s still a jack-of-all-trades like standard Spinda, this form of Spinda is different in that it doesn’t completely suck at everything. Special sets with Sludge Wave, Psychic and Shadow Ball could work, or physical sets with Knock Off, Gunk Shot, Wild Charge and Rock Slide. It’s still not the best thing in the world, but Tinted Lens makes it way more viable than standard Spinda will ever be.

UB-295 Subwoofer (Exploud)
Base Stats: 83/83/83/179/83/59 (570)
Type: Normal/Electric
Ability: Amplifier - 1.5x power to sound-based moves. (HA: Soundproof)
Removed Moves: Nothing.
Added Moves: Thunderclap (90 power, Electric type, special, 100% accuracy, sound-based, no special effects, hits all adjacent Pokémon), Thunderbolt, Thunder, Charge Beam, Discharge, Thunder Wave, Parting Shot, Chatter, Noble Roar
Description: Exploud wound up on a planet with a very thin atmosphere, where even its powerful shouts were barely audible. As a result, it evolved to become even louder, and is now capable of blasting opponents to smithereens by shouting alone. The planet was filled with strange piezoelectric crystals that produce an electrical current when force is applied to them. Exploud has made use of this, integrating the crystals into its body to power the two huge amplifiers on either side of its head. When it shouts, the crystals are activated and the amplifiers turn its already-powerful shouts into weapons of mass destruction.
Justification: Based on electric speakers. It’s basically Exploud, only even Explouder.
Competitive Analysis: This thing is the wallbreaker to end all wallbreakers. With an incredible 179 base Special Attack and an ability that allows it to power up its already-powerful STAB Boomburst and Thunderclap, this is a Pokémon not much can stand against. Slap some Specs on it and its Amplifier-boosted Boomburst can take a huge chunk out of even Chansey. Unless your opponent has something resistant against both of its STAB types or monumentally bulky, Exploud is going to eat their team for breakfast.

UB-713 Mountain (Avalugg)
Base Stats: 83/107/167/97/113/3 (570)
Type: Rock
Ability: Solid Rock (HA: Sand Force)
Removed Moves: All Ice-type moves except Avalanche and Mist
Added Moves: Stealth Rock, Sandstorm, Rock Wrecker, Heavy Slam, Precipice Blades
Description: Avalugg ended up on a desert planet, which was very bad for its icy body. Over time, its body changed from ice to rock and inverted, so the flat side is now its underside rather than the top. This form of Avalugg eats sand and rocks, adding the minerals to its body to increase its size. They continue to grow throughout their lives and live for a very long time. Some Avalugg are thought to be hundreds of thousands of years old and are the size of mountains.
Justification: Mountains are bigger than glaciers, but also slower. That’s all I got.
Competitive Analysis: Did I say that Pyukumuku, Shuckle, Munchlax and UB-Foundry were the slowest Pokémon in the game? Well, I must have been mistaken. Avalugg is as slow as an actual mountain and as bulky as one too. Its Defense might be a little lower than standard Avalugg, but it makes up for that with base 113 Special Defense. With good attacking stats, phenomenal bulk and the lowest Speed in the game, Avalugg is the star of Trick Room teams. Precipice Blades and Stone Edge are going to do a lot of damage to a lot of things, and with minimum Speed investment, Gyro Ball gets 150 base power on everything except other Trick Room or Gyro Ball users (and even most of those).

Description: Kecleon found itself deposited in an abandoned factory for making TMs. It stumbled upon a huge store of experimental TMs that were never mass-produced, which had the data for nearly all known signature or near-signature moves. Eventually, Kecleon incorporated the experimental TMs into its own movepool, giving it access to many extremely powerful and rare moves.
Justification: I wanted to make something with all the signature moves, and Smeargle was too obvious.
Competitive Analysis: With the insane amount of powerful moves Kecleon gets, an excellent ability in Protean, setup moves like Shift Gear, Z-Conversion, Z-Purify and the terrifying Z-Geomancy, and the world’s most severe case of 4-Moveslot Syndrome, this Pokémon looks like it came straight out of Hackmons. Sweep your opponent physically or specially, set up or steal the opponent’s boosts for yourself, or take advantage of Kecleon’s defenses and access to moves like King’s Shield, Parting Shot and a plethora of recovery moves to stall out all but the absolute strongest of opponents. I wanted to make some suggested sets for this, but since it can do basically anything, I can’t really narrow it down to four moves for a set.

Abilities: Lord Helix: Secondary effects always happens when used by this mon, aside from Freeze and Flinch, they are doubled chance rather then 100%.
Appearance: The tentacle things become wings and the moth becomes a beak. Body color is white.
Possible moveset(s): Ancient Power/Shell Smash/Air Slash/Earth Power seems the most logical to me. OP as hell hability, good offensive typing, good offenses. Has Shell Smash too.

Possible moveset(s): Best support mon ever. Dual Screens, okay special offense, fantastic defenses, wide pool, and that hability man. You chose which way you want to use as support

Abilities: Cryogenic Aura (Lowers Speed upon contact. All contact moves used by this Pokemon or the foe have a 30% chance to freeze)

Appearance: The tree on Torterra's back sheds all of it's leaves, and it's body turns a dark purple instead of brown.

Name: Ultra Mimikyu (UB X3X Hidden Sketcher)

Abilities: Hidden (For the first turn upon switching, the foe will always miss their attack.)

Its base HP can still be 2, but the raw number can be hard-coded to 1. That's how it is in the games and on Showdown, and I'm fairly certain that if Game Freak ever made this monster they'd do the same.

Appearance: Azumarill is now yellow. The bubbles are now lightning bolts. Just imagine a Dark/Electric Azumarill and that's basically it. I'm not very good at imagining this kinda stuff.
Lore: This Azumarill adapted incredibly well to Ultra Space, although its biology changed incredibly drastically. It adapted so well, in fact, that it is able to use attacking moves of almost every type! Unlike normal Azumarill, which prefer to attack up close, UB-184 sets barrages of long-ranged attacks at foes.
Competitive Role: Hit everything super hard! Great bulk, great power, priority for most types, what more could you want?

I was hoping it would stay at 1HP which is why I did this. This is just a take on Sturdinja. Besides, this things survives attacks, not entry hazards
 
Let's just say the species mutated diferently or whatever.

Evolution Method: Level up to Lv. 37 and teach it Stored Power, or level up while knowing Stored Power
Appearance: Only a few Magikarp were ever shot into Ultra Space, all of them by accident. A careless Aether Foundation employee was using an empty test rocket to act as an aquarium for his pet Magikarp, and when Ultra Wormhole opened up nearby he panicked and hit the launch button. The harsh environment of Ultra Space proved too much for the fish, turning them to stone, but it ended up giving them odd powers. While the Magikarp body is lifeless, the restless spirit of a Gyarados remains inside, constantly struggling to break free.
Competitive: Stored Power is a move that doesn't see much usage in competitive play, so I decided this thing would be the best bet to make use of it. Good bulk means it can set up easily, and reliable recovery means it can stay in the fight for longer.

Appearance: After breaking out of its rocky cocoon, Gyarados becomes a nearly unstoppable force. While its physical body is gone, its spirit has manifested itself into a creature resembling an astral dragon. It often preys on Nihilego.
Competitive: Set up Dragon Dance or Nasty Plot, then sweep. Easy as that.
