Issues I can think of:
-did not see gale's LoA in the beginning
-passed dq by like 2 weeks on the first reffing
-a few errors in the reffing itself early on
-formatting things gale told me to do
-consistent dq errors again :(

The fact that you are aware of which things you have to work on, is a really good place to start. Reffing and ordering within DQ is THE most important thing. Everything else can be learned and is learned by most players everyday.

There was considerable progress by you in the last few rounds, and you started off pretty well. Imo, reffing doubles in general and the way the battle was going might have demotivated you a little in the middle. But the fact that you bounced back on reffing time indicates that it is not a consistent negligence of DQ.

Asking you to work on the things that you have learned from this battle is something that I think is acceptable. But white-listing you, and monitoring your battles wouldn't be a bad idea either.

Requesting Frosty to take the call in this, because either way I think you will only get better if you keep working on your DQ times.
 
We also forgot to mention that the gym match i took for this hasn't finished because the first one I took ended in a forfeit.

I told Gale some time ago of this conclusion, but I suppose I gotta make this official.

Your disregard for DQ, spending up to 10 days to ref means that you are automatically rejected. I need to be sure you can keep yourself to DQ and take it seriously when necessary. You didn't show me that (in fact you showed me the EXACT opposite). So the final verdict is No.

You can reapply in one month. I suggest that when you do so, you take this a bit more seriously.
