Battle Mistakes:
1) Imprison is self-targeting. You can't really target gardevoir with it. Although you reffed it just fine, so this is minor.
Reffing Mistakes:
NOTE: There were other listed. But I took only the ones that I felt mattered in some degree.
2) Struggle randomises between the target(s) of the move that would've been used, and is single target. This means that it should have randomly targetted one of Tomohawk and Cofagrigus, rather than hitting all 3 mons.
3) Struggle's Recoil is 40%. On a 3 DMG hit, that should've had 1.2 recoil.
4) Please mark Intimidate on your team's stats
5) My (df) current maths has Cofagrigus only take 31.25 ~ 31 damage
6) Tomohawk's Heat Wave vs Sylveon wants a Burn Roll
7) Why does Gallade have a +3 on it's Night Slash?
8) Cofagrigus should have a +2 on it's Shadow Ball
9) Most importantly, how did Sylveon's A2 Hyper Voice have a Fairy Typing when Sylveon's ability was Mummy?
10) Remember your combo priorities. Moonblast + Moonblast, alongside Round + Synchronoise, both are priority -2. This means that Pory-Z should act before them.
11) Speed Ties are resolved the same way in Trick Room as out of it, which means that Gallade's lower EN Usage in the same prio tier makes him act first. Which is a little bit awkward, but it's sort of like how -2 prio moves don't go first :P

14) Porygon-Z's second sub can't activate. Since Skull Bash charges at +1 Prio, and Pory-Z's sub tries to make the check at +0 prio (which is when it acts on all 3 actions), the sub would try to make Pory-Z move back in time.

My rule of thumb is 15. If you have 15 or more mistakes total, I will be inclined to fail you depending on the potential repercussions of those mistakes and how mean your mentor was (the meaner the mentor, the more tolerant of mistakes I'll be). If that number is bigger (like 18 onwards) I will just outright fail you.
You made...14 mistakes. Two of them minor. And your mentor was the 202's version of a grammar nazi. While that number of mistakes IS considerable, it isn't beyond what I would expect from a decent ref on a situation like this. Walking on very thin ice though.
So:
Verdict: Consider yourself whitelisted. To a degree. I will be watching your matches and I expect to see you working on those numbers. If I feel you are making the same mistakes over and over, I may revisit the matter at some point.
This white-listing expires in 6 months. You need to pick a reffing 202 to extend it for more 6 months from the day you picked it. BUT,
don't pick any reffing 202 mentorship for the next 4 months. I don't think you are 100% ready for it right now. If you feel you are ready to get them, let me know and I may check it out. But for now I suggest you spend the next 4 months getting a better grip at ASB's intricacies and random details. Worry about renewing your licence from the 5th month onwards.
