So I finally got around to cave abusing today.
I forgot to set my chatter chatots, fail!
But I found a cave spot. It seems as if the RNG calls for the items and synch at the same time. In which case the item is called over the synch, so at any synch frame an item appears. So for cave abuse I have to abuse for a PID frame that does not ask for a synch at all.
