I don't think there are any rules regarding avatars other than what applies to just forum presence in general; keep it appropriate. There are kids here I don't believe there are any resources like that on Smogon. I would check on Pokemon Showdown for one but I'm not sure.

Well if it is a big image then yeah it'll probably get cut off. Mine was sized perfectly I think it was between 400-600 px by 400-600 px.

The problem could be what Blackdrakon30 stated OR it could be that the computer's cookies are being cleared. When that happens, it gets rid of all your teams, your avatar, and your choice of background (whether it's custom or not).
