We're trying out a new mod called "Heroic Death" which pretty much just broadcasts a message when someone dies:

We can add plenty more, there are currently on about 2-4 different messages for each death type.. if you want to suggest something, post them in the following format:

Examples
Death Type: Creeper
Message: %d hugged a creeper

Death Type: PvP
Message: %d was DECIMATED by %a wielding their trusty %i

Where:
Death Types: Drown, Cactus, Fire, Explosion, Creeper, Fall, PvP, Void, Monsters, Lava, Other, Suffocation, Dispenser, Lightning

%d is the player who dies
%a is the attacking player in a PvP kill.
%i is the item the attacking player is wielding in a PvP kill.

That's probably because the server is lagging a bit and didn't load all the textures for that particular patch of blocks. Usually it goes away before you can get that close, but sometimes it just gets stuck. Logging out and back in should fix it.

Incidentaally, what else qualifies "Other"? Is that like a creeper falling onto you cand causing you damage?

I think it's a catch-all in case Notch adds some other way to die so the server doesn't crash when something happens.

If an op uses /kill it falls under Other. Can't think of anything other than what Aura Guardian said.
